Sliding Scale Self-Pay PatientsOnly

A sliding fee scale is a model for payments administered to patients. The scale is intended for our patients who can’t afford care to begin with, including those at a lower income level, and self-payers. The sliding scale makes giving you services more affordable. Below are the items we need to approve the sliding scale.
Proof of IncomeDocuments
We accept the following documents as proof of income to qualify for the sliding fee discount:
- Paycheck Stub- The most recent paycheck stub of all employed persons and persons receiving other income in the household.
- Income Tax Return – The most recent income tax return for all employed household members.
- Agency Letter – Letters from local, state or Federal agency (Social Security Agency or Veteran’s Administration) must state the monthly income before deductions.
- Unemployment Agency – A letter from the unemployment agency.
- Letter from Employer – A letter from employer detailing current gross income and frequency of pay periods may be used for those patients/household members who are paid in cash. The letter must be on the employer’s letterhead (unless non-applicable), contain the employer’s signature as well as telephone number for verification of supplied information.
- Fixed income statement – Pension or bank statements showing deposits.
Once received we will notify you within 48 hours and your payment will be due 48hrs prior to your appointment if approved.
What to Expect at Your First Appointment
If you decide to pursue treatment with us, here’s an overview of what you can expect at your initial appointment:
· You’ll first complete some brief paperwork asking about your symptoms, medical history, and treatment goals.
· Then you’ll meet with a psychiatrist or psychiatric nurse practitioner for an in-depth evaluation. They’ll ask detailed questions about what you’re experiencing and how depression is impacting your life.
· Certain assessments may be given, like a depression screening quiz or anxiety inventory. This further reveals the nature and severity of your symptoms.
· Any lab tests needed to uncover potential biological causes will be ordered. A physical exam may also be completed.
· By the end, our provider will summarize their initial recommendations for medication, therapy, and next steps in your treatment journey.
· You’ll have plenty of time to ask questions and address any concerns before moving forward.
· Initial appointments typically last 60-90 minutes so we can gain a thorough understanding of your needs and set you up for success.
